304 - Premises not free of pests [s. 26(a)] Observation: -six+ live adult cockroaches found in the main kitchen area -multiple nymph cockroaches found throughout the kitchen (back area, dishwashing area, and hot line) -multiple dead cockroaches found in the glue traps particularly around the dishwasher -dead cockroaches found in mixer attachment basket in the basement kitchen -live and dead cockroaches found in furnace room in the basement -multiple live cockroaches found in the bar (near electrical outlets) -three fruit flies found in bottle of Aperol -live adult cockroach found near the ice machine in the basement Corrective Action(s): set glue traps in the basement furnace room, around the ice machine, around the dishwasher, around the printers, and around the basement 2 compartment sink remove all dead cockroaches reset sticky traps for monitoring (underneath the dishwasher) clean behind the wooden counter tops in the basement Violation Score: 9 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)] Observation: -Grease build up observed above the canopy in the kitchen, numerous cockroaches stuck in grease -food debris noted in seasoning shelf in the basement kitchen -two detergent bottles underneath the dishwasher has debris build up -24 hour fan above the upright coolers found to have dust and grease build up -various debris catch trays and their receiving areas on the grills and stoves have heavy food debris build up inside Corrective Action(s): Thoroughly clean the above noted areas Violation Score: 9
