Observation: The following pest activities were noted:
1. number of dead cockroaches at prep area floor.
2. 1 live cockroach near the dishwasher area.
3. approximately 20 live cockroaches (mostly nymps that are 1-2mm in size, 1-2 adults observed) in the dimsum station floor.
Corrective Action(s): The facility must be free of pest activity. Correct this by: October 7, 2022.

Observation: 1. (Repeated violation) observed number of in-use towels on the counter that are moist.
2. (Repeated violation) back door was not closed properly that a large gap was present.
3. (Repeated violation) standing water observed throughout the facility.
4. (Repeated violation) Sanitation issue noted with floor drains, behind the main cook line and under the dimsum steam table areas.
Corrective Action(s): 1. Every in-use towels are to be stored in a sanitizer (100 ppm chlorine) when not in use.
2. Fully close the back door when not in use.
3. Remove standing water. Fix any issues that may contributes to water leaking.
4. Clean and sanitize areas described in the observation section. Eliminate food, grease and standing water.
Correct this by: October 7, 2022.

The undercounter cooler at the noodle bar was serviced and measured at 4C or less.
