Observation: Counters, shelves, floors, fridge handles, walls, ceilings: all covered in hardened food debris and dirt. Most surfaces do not appear to have been cleaned for some time. Many counter surfaces and fridge handles were sticky.
Corrective Action(s): Clean all above surfaces thoroughly.

Observation: Multiple food items not covered in walk in cooler and cooler racks are covered in caked on dirt and rust right above the uncovered food.
Corrective Action(s): All food items must be covered until the cooler racks have been properly cleaned and scrubbe.

304 - Premises not free of pests [s. 26(a)]
Observation: 1) Rodent droppings observed on floors throughout dry storage. Flour had spilled onto the floor along the side wall, and rodent droppings observed throughout the flour
2) Flies throughout restaurant. While walking from kitchen to dry storage, flies were observed throughout entire kitchen.
Corrective Action(s): 1) Remove all droppings and sanitize all affected areas.
2) Contact pest control and get visit
3) Keep a copy of the report for the follow up inspection

Observation: Sanitation throughout kitchen is very poor:
1) Sticky surfaces throughout kitchen (freezer tops, fridge handles, utensils storage shelves, underneath counters (blackened grease dripping down onto food containers)
2) Cobwebs throughout shelving units in the back. Cobwebs covering bags of tea leaves, cobwebs throughout dry storage as well and in many corners of restaurant, indicating kitchen has not been cleaned regularly
3) Walk in cooler racks covered in old food/dirt/debris
4) Floor underneath grill has a dirt pile towards the back (most likely from accumulated grease buildup over time)
Corrective Action(s): Closure order is issued for poor sanitation throughout (amongst other violations).
All above areas must be cleaned immediately.

Observation: Multiple shelves throughout kitchen (with dishes, teapots, bags of tea, other food items) stored on top are in poor condition. Some have rotting wood and others are covered with hardened dirt and debris that they cannot be cleaned.
Corrective Action(s): 1) Remove all food items and utensils from rotting/pitted shelves
2) Replace any shelving that cannot be cleaned. Surfaces for all shelves should be smooth, non porous

Restaurant must remain closed until re-inspection is conducted.
