Observation (CORRECTED DURING INSPECTION): Soiled wiping rags in use without sanitizer solution prepared.
Corrective Action(s): All wiping cloth rags in use are to be stored in a container of sanitizer solution (200 ppm Quaternary Ammonia, to be verified by test strip).

Observation (CORRECTED DURING INSPECTION): Peanut butter inventory stored on the floor with chemical products.

Wheat grass spatula stored in a container of stagnant water.

Large volume of khale stored in white garbage bag.
Corrective Action(s): Food items / food containers are to be stored off the floor. Foods should be placed on shelves that provide 6” clearance from the floor.
Do not store utensils in use in water. Food utensils in use can be stored in a dry container or a container filled with ice.

Do not store food items in garbage bags. Food products must be stored in suitable food grade packaging.

Observation: Front entrance swing gate - door sweep on right side has gap allowing entry of mice into premise.
Corrective Action(s): Adjust or replace door sweep to exclude pest entry be eliminating gaps between gate and adjacent wall.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Mouse droppings observed on floor throughout entire premise, including the following areas: behind ice machine, staff storage corner, under green wire shelving in storage room, around step ladder storage area and under / behind all counters / coolers / freezers in front service area.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.

Premise cleaning is to be conducted on a daily basis throughout premise to maintain sanitation and discourage pest activity.

Failure to maintain premise sanitation will result in premise closure.
