Observation (CORRECTED DURING INSPECTION): A container of cooked bacon was found at room temperature (22 deg. C).
Operator was instructed to put container of bacon into the cooler at below 4 deg. C as operator indicated less than 2 hours ago.
Corrective Action(s): Keep potentially hazardous foods at 4 deg. C or colder or 60 deg. C or hotter.

Observation: There was black residue found on the side wall of the ice machine (inside) and pink mold observed on the part at the back of the inside of the ice machine. Operator mentioned that is not used for consumption, only to put below the food bowls at the self-serve station. However, it was observed at time of inspection that the ice occasionally fall into the food.
Corrective Action(s): Remove and discard ice. Clean the ice machine as per manufacturer's instructions as required.

Observation: The was no lid on the ice machine. It appeared that the lid was broken off.
Corrective Action(s): Repair or replace the lid so that ice is protected from contamination.

Comments

General Comments:
-Glass cooler at back was 9 deg. C at beginning of inspection. It went down to 4 deg. C at end of inspection. Continue to monitor temperatures.
-Tomatoes in insert above cooler in front was 8.8 deg. C. Operator was instructed to put the insert in cooler below at 4 deg. C.
-double door refrigerator at front at 4 deg. C
-Coca-cola cooler at 4 deg. C
-Aquafina cooler at front 5 deg. C (only held bottled beverages)
-freezer was -12 deg. C
-Dishwasher was 76.8 deg. C on the plate.
-Gravy at 61 deg. C
-Other hot held items at 60 deg. C - 73 deg. C
-The eggs at self serve station station was measured 4.5 deg. C, cottage cheese at 4 deg. C, cooked veggies at 6 deg. C. Only small quantities were present for limited time. Ensure containers are fully submerged in ice and temperatures monitored regularly (cold-holding of potentially hazardous foods should be kept at 4 deg. C).
-Fume hood / canopy was free of grease.
-Bleach sanitizer solution >>200 ppm. The sanitizer solution in spray bottle was diluted to 100 ppm - 200 ppm. Test strips was present. Use test strips to verify concentration. 2 ml of bleach per 1L of water to make 100 ppm of bleach solution.
