Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care & Assisted Living Act (CCALA), the Child Care Licensing Regulation (CCLR) and the relevant Director of Licensing Standards of Practice (DOLSP). Evidence for this report was based on the Licensing Officer’s observations, review of the facility records and information provided by the facility staff at the time of inspection.
The following areas were reviewed:

As part of the routine inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection and a 3 year “historical” review of the facility’s compliance and operation.

Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11010 - CCLR s.13(1) - A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation (CORRECTED DURING INSPECTION): A kettle (for boiling hot water) was put on top of a small refrigerator. Children has access and can reach the kettle.
Corrective Action(s): Please ensure that children do not have access or cannot reach the kettle to prevent scalding.
Recommend to put it into the storage room .

Date to be Corrected: March 14, 2018. Corrected during inspection .

STAFFING: 12140 - CCLR s.19(4)(a) - A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications].
Observation: Manager's file were reviewed. The proof of ECE certificate validation was not found.
Corrective Action(s): As per the regulation, you are required to verify the ECE certificates via the ECE registry website.

STAFFING: 12180 - CCLR s.22(2)(a) - A licensee must ensure that each employee (a) is trained in the implementation of the fire drill system and emergency plan described in subsection (1), including the use of any equipment noted in the fire drill system and emergency plan.
Observation: The fire drill procedure was briefly discussed. The manager stated that they dressed the children and directed children to outdoor away from the facility during a fire drill.

The next service of the fire extinguisher was in Nov 25, 2017.

Corrective Action(s): Please review the fire drill and emergency plan. Staff should not dress the children during fire drill instead they should direct children out to the outdoor immediately.

Please ensure that the fire extinguisher is serviced according to the manufacturer's instruction.

POLICIES AND PROCEDURES: 13071 - CCLR s.56(2) - A licensee must ensure that the policies and procedures referred to in subsection (1) are implemented by employees.
Observation: Two children's emergency supplies were not provided by the parents. The manager stated that they had talked to the parents but they did not provide.
Corrective Action(s): Please ensure that the facility has sufficient emergency supplies on site for all the children during emergency situation.

POLICIES AND PROCEDURES: 13090 - CCALA s.7(1)(b)(i) - A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,
Observation: the Active Play and screen use policies were briefly discussed. It appeared that the manager were not familiar with the Standard of practice requirements and the policies were not developed . The manager stated that they don't use any screen at the facility.
Corrective Action(s): Please refer to resources that I had sent via emails and developed and implement Active Play and Screen Use policies. A copy is shared with the parents and families.

HYGIENE AND COMMUNICABLE DISEASE: 15050 - CCALA s.7(1)(b)(i) - A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,
Observation: I noted some children's blankets and pillows were stacked up in a crib. The manager explained that they did put them separately into their bags , however, children might take them out or put them back into the crib.
Corrective Action(s): Please ensure each children's personal items are stored separately.

RECORDS AND REPORTING: 19050 - CCLR s.56(1)(f) - A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Observation: Minor incidents were recorded on some lose papers. Year of the incidents and the follow up to the incidents were not recorded.
Corrective Action(s): Please ensure that you record all the incidents with details. Recommend to record them on a book so you will not lost or misplace them.

RECORDS AND REPORTING: 19070 - CCLR s.57(2)(a) - A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation: 6 children's files were reviewed. 3 children's medical insurance plan numbers were missing from file.
Corrective Action(s): Please ensure you have obtained all the required information.

RECORDS AND REPORTING: 19100 - CCLR s.57(2)(d) - A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation: 6 children's files were reviewed. two children's medical practitioner's information were missing from files.
Corrective Action(s): Please ensure you have obtained all the required information

RECORDS AND REPORTING: 19160 - CCLR s.57(2)(i) - A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation: 6 children's files were reviewed. 3 children's photo were missing from files. One photo appeared to be taken when they were younger and one photo has two children in them.
Corrective Action(s): Please ensure that all children's photo's are current and each child should have their own photo in file to avoid confusion.

RECORDS AND REPORTING: 19190 - CCLR s.57(3)(a) - A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.
Observation: 6 children's files were reviewed. 2 children's emergency card were missing and two children's consent cards were not completed . Two children were using one consent card.
Corrective Action(s): Please ensure each child has their consent card . You must ensure that the parent has completed and provided you all the information.

RECORDS AND REPORTING: 19320 - CCLR s.59(a) - In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation: Manager's file were reviewed. the Criminal record check results and immunization status were missing from file.
Corrective Action(s): Please ensure that all the required documents were kept in the facility.

Comments

The hot water in the bathroom sink was checked. It appeared that there was no hot water or warm water. The manager stated that they have notified and asked the Licensee to fix it.
Warm water should be available in the bathroom sink so that children can wash their hands properly.

Children were playing in the garage during the inspection. I witnessed that children fell from a bike at least two times during my observation. This bike's seat appeared to be too high for them. The manager agreed to adjust the height of the seat.

Please provide a written response by April 14, 2018 on how the above contraventions items have been addressed.
