Observation: Mouse droppings on shelves, cupboards and food preparation surfaces, and in dining area.
Corrective Action(s): Clean up all mouse droppings and sanitize all affected areas with bleach and water (1 oz bleach per 4 litres of water). All dishwash and utensils in these areas shall be washed in the dishwasher before they are re-used.

Observation: Storage/staff room and wall cubby by the washrooms are cluttered.
Corrective Action(s): Organize the storage areas and remove anything that are not required for the operation of the business. Cluttered storage areas provide areas for mice to hide and breed.

Observation: Sanitation of premise is poor, especially in the kitchen and food preparation areas.
Corrective Action(s): Clean the floors and walls of the kitchen, dry storage room, walk-in cooler, servery area and sushi bar area. All shelves and cabinets shall be cleaned. All areas of the dining area shall also be cleaned.

Comments

Closure order issued due to abundance of mouse droppings throughout the premise and poor sanitation.

After all of the cleaning has been completed, the services of a licensed pest control company shall be retained to assist in rodent control. The services of this licensed pest control company shall be retained for a period of no less than 6 months.
Provide a copy of the pest control company's first work report and a copy of the 6 month contract for review.

Re-inspection is required before re-opening.



General observations:
All cold holding units were at the appropriate temperatures.
Hot holding units were over 60 degrees Celsius.
Hand sinks stocked with liquid soap and paper towels.
All sinks equipped with hot and cold running water.
High temperature dishwasher had a final rinse temperature of at least 71 degrees Celsius (as per thermal label).
Bleach sanitizer was 100ppm chlorine.
