Corrective Action(s): Do not place bus pan full of raw chicken above any ready to eat foods such as rice and salad vegetables ( Immediatley)

304 - Premises not free of pests [s. 26(a)]
Observation: A number of mice droppings noticed in the old comressor area of the decommissioned cooler. Some droppings also noticed behind the pizza pan storage area, underneath the dishwasher and oven. Glue traps are being used by the operator for mice control. A licensed pest contorl company is on contract.
Corrective Action(s): Clean and sanitize all of the above areas to remove all droppings. Ensure that for the next week weeks, floors and all other surfaces are kept abosolutely free of all food debris and water (Dec. 10, 2018).

Observation: Food debris noticed behind and underneath the pizza ovens, pizza pan storage cart, dishwashing area and compressor cabinet.
Corrective Action(s): Improve general sanitation in all areas of the restaurant. Ensure to seal all gaps, holes and cracks in the walls to discourage pest harborage. Ensure to clean thoroughly every night to remove debris from all hard to each areas. ( Immediately).

* More through and frequent cleaning required to discourage mice.
* Pressure washing recommended to clean the floors thoroughly.
* Failure to take prompt actions will result in a Correction or Closure Order.

* Do not use ordinary plastic buckets to cool hot meat sauce. Meat sauce must be stored or cooled in
either stainless steel or aluminum cooking pots or NSF ( rated safe to 99 degrees C) plastic pans only.


No signs of pests noticed in the dry storage room. All items are not stored at 18 inches off the floor.
Continue to monitor all areas of the restaurant for signs of pest activity.
