Corrective Action(s): Ensure back door remains closed at all times to prevent possible pest entry directly into kitchen. Install screen door if back door is to remain open. Closed back door.

Observation: Staff washroom sliding pocket door difficult to open and close. Door fallen off track. Door handles not are replaced by zap straps through mounting holes.
Corrective Action(s): Ensure door opens and closes properly. Replace temp zap strap handles with proper solid handles permanently mounted and easy to clean.

-Handsink and staff washroom supplied with hot & cold water, liquid soap and paper towel
***Reduce build up of toilet paper rolls in staff washroom. Restock tp dispenser, remove extra used rolls.
- Dishwasher noted: 50 ppm chlorine at the dish
-Sanitizer bottles through out facility: 200 ppm QUATS
***Bar sanitizer bucket noted 100 ppm QUATS after rush - replenish sanitizer solution after lunch rush
-Food appears protected: covered and at least 6 inch off the ground
-Ice machine and scooper maintained in sanitary condition
-Meat slicer stored on shelf clean in sanitary condition
-No signs of pest activity noted - pest control done by Aggressive pest control - reports reviewed last 5 months - no concerns
-General sanitation satisfactory - Fume hood appears clean, food prep surfaces
