Observation: Rat droppings found in back storage area on floor behind freezer units.

Corrective Action(s): Spray rodent droppings with 200ppm chlorine solution or equivalent. Remove droppings from area. Wet affected area with 200ppm chlorine or equivalent and soak area.
Inspect facility for points of entry for pests. Seal any holes.
Obtain services of professional pest control company.
Have traps set.

Observation: Holes found in lower portion of North wall inside storage room on back of restaurant.

Corrective Action(s): Pull all freezers out and seal all holes with non-chewable material such as wood.
Spray foam insulation has been used however it is coming apart and other areas of drywall have holes. Seal entire lower portion of wall area with non-chewable material such as wood or metal sheeting.

Observation: Much cleaning of restaurant required.

Corrective Action(s): Extensive cleaning required. The following is a list of areas which require cleaning however it is not a complete list as other areas will inevitably require cleaning as well:
- Shelves
- Microwave interior
- Backsplash/walls throughout
- Inside coolers including cooler seals, handles, shelves and walls
- Floors
- Dough mixer surface
- Bubble tea sealer and area
- Entire back storage area
- Remove all items unnecessary for business. Ensure all items are stored up off floor. Discard any broken or extra items. Much clutter exists at facility.

- Sanitizer solution in use throughout food preparation areas and at concentrations of greater than or equal to 200ppm Chlorine - EXCELLENT WORK.
- Two functional sink plugs available for use observed
- Ice machine in good sanitary condition
