Observation (CORRECTED DURING INSPECTION): Noted 3 small containers of rice stored at room temperature. Measured at 25C. Per staff, rice is to be taken from the cooler and reheated immediately.
-Staff discarded rice at the time of the inspection.
-Ensure that cold potentially hazardous foods are stored at or below 4C.

Observation (CORRECTED DURING INSPECTION): 1) Ice machine had mildew and dust build-up at the back and side areas.

2) Waffle maker stored in undercounter shelving had dried food-debris.
Corrective Action(s): 1) Discard ice. Clean and sanitize ice machine.
-Ensure ice machine is cleaned and sanitized on a regular basis as per your Sanitation Plan.

Observation (CORRECTED DURING INSPECTION): Handsink in front area was blocked with a teacup and utensil.
-Items were removed from handsink. Instructed to move dishes to dishwashing area for cleaning.
-Handsinks are for handwashing only. Ensure handsinks are available at all times for staff to properly and frequently wash their hands.

Observation (CORRECTED DURING INSPECTION): Metal ice scoop was stored inside ice machine. Handle of scoop was buried in the ice. This is not a sanitary way to store ice scoop as the handle can contaminate the ice.
Corrective Action(s): See Code 301.
-Ensure ice scoop handle is not stored inside the ice. Recommend storing ice scoop in a clean container outside the machine.

Observation: Alterations to original floor plan and menu items noted at the time of inspection. Additional tabletop equipment noted - ex. rice cooker, instapot, heating units. Additional menu items noted - ex. rice bowls, pre-cooked chicken, waffles. This is a repeat violation.
-Per file, premises is approved for bubble tea, tea/coffee, and baked goods.
-Staff states that an updated Food Safety Plan, menu list, and floor plan was sent to the district inspector after the last inspection. Staff denies receiving confirmation of receipt. Staff could not provide any written health approval of updates.
Corrective Action(s): Return premises to original floor plan and menu items that was granted approval.
-Submit proposed floor plan, food safety plan, and menu list to district inspector for review. Health approval is needed before making any changes to premises.
-Non-compliance will result in further enforcement action.

-Container of bubble tea pearls was placed at room temperature less than 1 hour ago at time of inspection per staff.
*NOTE: Discussed room temperature holding with staff. Potentially hazardous foods stored at room temperature for more than 2 hours are to be discarded. Recommended to use time stamp or timer.

-Handsinks have hot/cold running water, liquid soap, and paper towels.
-High-temperature dishwasher is adequately sanitizing dishes. Measured 78C at plate surface.
-Sanitizer solution available in back and front areas. Measured at 200ppm chlorine using test strips.
-No signs of pest activity at time of inspection. Building provides pest control services.
-Overall sanitation is satisfactory.
-FoodSafe certified staff on duty. Verified at time of inspection.

-Signature not obtained due to COVID protocols. Report printed and reviewed with staff on-site.
