Frozen foods are to be thawed safely in a cooler unit at 4 C, in a clean sink under flowing cold water or using a microwave (small portions only).

Observation: Food insert containers and lids stored in milk crate on floor on top of mouse droppings.

Food utensils stored in heavily soiled containers in back kitchen, under front rice warmer counter
Corrective Action(s): All food utensils are to be re-cleaned and sanitzed through the dishwasher.

All food utensil containers are to be cleaned and sanitized.

Cleaned, sanitized food equipment and utensils are to be stored in a sanitary location that protects them from contamination prior to their next use.

Observation: Multiple wiping rags on counter tops with no detectable sanitizer residual.
Corrective Action(s): All wiping cloth rags in use are to be stored in a container of sanitizer solution (100 ppm chlorine, 1 tea spoon of bleach per 1 litre of water).

Observation: Vegetables, fresh noodles stacked upon containers of raw meat in upright cooler unit.

Staff handling food items at 2 compartment sink next to cleaning chemicals and soiled dishware.

Open containers of sesame seeds stored in cabinet with active cockroach infestation.

Corrective Action(s): Raw meats and eggs are to be stored below and away from contacting potentially ready to eat food items such as vegetables.

Staff are not to conduct any foodhandling at 2 compartment sink when sinks are full of soiled dishware and cleaning products are located next to food.

Dry food items are to be stored in sealed pest proof containers.

Corrective Action(s): Pest control service will conduct an evaluation of current pest activity and employ additional pesticide to eliminate cockroach activity in kitchen and storage room and apply additional pest proof measures as needed.

Observation: Premise sanitation is extremely poor.

Back kitchen upper wall shelving dust collecting on surfaces by stove.

Front kitchen grease and food debris build-up under grill area and space adjacent to wall.

Mouse dropping build-up observed in the following areas:

Back kitchen - on all levels of black plastic cart, brown sugar bin, right swing gate door, under dishwashing sinks, on stove, on floor behind stove, and on clean dishware storage shelving below meat slicer.

Front kitchen - on floor along entire front service counter, under back storage cabinets, and on take-out container storage shelf.

2 dead dessicated mice under front service counter.

Cockroaches live and dead observed in back kitchen on wheeled cart, throughout under counter cabinet next to beverage dispenser and beside / behind chest freezer in storage room.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.
