Observation (CORRECTED DURING INSPECTION): Spaghetti (28C), pancit (29C), steamed bokchoy (17C), and chopped pork left out of the hot holder and stored at room temperature, no time tracking for foods.
Corrective Action(s): discarded. operator indicates that the hot holding unit is too hot and dries out food; must be kept at room temperature.
All hot potentially hazardous foods must be kept at 60C or greater

front of house: handsink was completely blocked by plywood and used as a garnish station
kitchen: handsink's soap dispenser was missing, operator is using hand sanitizer
Corrective Action(s): ensure that all handsinks are stocked fully with liquid soap, paper towels, and hot/cold running water. Handsinks cannot be blocked

Observation: live adult cockroaches observed in the back of house near the kitchen handsink and around water supply line (behind sliding mini door). dead cockroaches observed on top of the dishwasher, underneath the wire shelving for pots/pans in the kitchen.
Corrective Action(s): have professional pest control service the facility
Operator MUST repair pooling water underneath the dishwasher and conduct a deep clean of the kitchen by pulling back equipment from the walls and cleaning behind them.

Observation: 2 issues;
-low temperature dishwasher measured 0 chlorine at the dish surface
-food processor was broken in several areas and repaired with painters tape and plastic tape.
Corrective Action(s): closure order issued
repair the dishwasher immediately. Dishwasher must be able to sanitize dishware with minimum 50 ppm chlorine at the dish surface.
Tape is not permitted as suitable for repairing the food processor. replace the unit

Observation: 2 issues;
-Backflow preventer near the hot water tank is rattling and leaking violently
-middle area 2 compartment domestic style sink stocked with sponge, dish soap, and a drying rack. this is not an area permitted for manual warewashing
Corrective Action(s): -service the backflow preventer immediately
-remove the sponge, dish soap, and drying rack from the area immediately
