Observation (CORRECTED DURING INSPECTION): Bleach sanitizer bottle in kitchen measured at >> 200 ppm chlorine concentration with test strip during inspection. Staff prepared fresh bleach sanitizer which measured at 200 ppm chlorine concentration with test strip.
Corrective Action(s): Prepare bleach sanitizer at a concentration of 100-200 ppm chlorine concentration by mixing 1/2 teaspoon bleach with 1L of water to adequately sanitize food contact surfaces and equipment. Obtain chlorine test strips and test sanitizer strength as per approved sanitation plan.

Observation (CORRECTED DURING INSPECTION): [REPEAT VIOLATION] Hand washing station in kitchen is observed obstructed by large garbage can. The hand sink basin is observed storing a pile of dirty dishes and utensils. Staff moved the garbage can away from the hand sink. Staff moved the dishes into the 3-compartment sink.
Corrective Action(s): DO NOT obstruct the hand washing station and DO NOT place any items in the handwashing sink basin. Ensure the hand washing station is unobstructed and supplied with hot and cold running water, liquid soap and paper towel at all times to ensure staff are washing their hands properly and at adequate frequency. Re-train all staff to wash their hands frequently. Failure to comply may lead to progressive enforcement action.

Observation: Multiple ants observed under the hand washing sink in the kitchen.
Corrective Action(s): Clean up all ants. Clean the premise to remove food/sticky residue. Place appropriate approved ant traps on the floors. Block off all entry points for ants (e.g. seal up all cracks in walls, fix broken tiles). Contact pest control company to assess ant population and apply treatment as necessary.

Observation: Food premise observed in unsanitary condition with food debris, dust and/or grease build-up in the following areas:
- Behind and around all cooking equipment
- Behind and around 3-compartment sink and hand washing station in kitchen
- All floors
- Wire shelving in dishwashing area
Corrective Action(s): Degrease, clean and sanitize the aforementioned areas to remove food debris, dust and/or grease and to prevent potential contamination of equipment and food. Ensure to deep clean the premise (especially hard to reach areas) frequently to prevent build-up of food.

Observation (CORRECTED DURING INSPECTION): Two bleach spray bottles observed not labelled during inspection. Staff labelled the bottles "bleach sanitizer" during inspection.

- Permit posted in a conspicuous location on site
