Observation: Quats dispenser and all quats buckets were at 0 ppm.
Corrective Action(s): The quats sanitizer solution was manually mixed to 200 ppm.

Observation: 1) Paper towel dispenser at one of the front hand sinks (near the sandwich table) is not placed in an easily accessible location and is not being used.
2) Quats dispenser is not working properly.
Corrective Action(s): 1) Install a new paper towel dispenser at this sink within one week.
2) Fix as soon as possible.

Comments

-Food Safety Plan not in use: for the past two days, temperatures above 4 C were recorded for the walk-in cooler and no corrective action was taken.
-Sanitation Plan not in use: All sanitizer buckets at the front had 0 ppm Quats. The dispenser at the 3-compartment sink had 0 ppm Quats.

All coolers were < 4 C.
All freezers were < -18 C.
Hot held items were > 60 C.
Hand sinks supplied with hot and cold water, soap and paper towels, with the exception of the above mentioned sink.
Dishwasher final sanitizing rinse reached 77 C at the plate level.
General sanitation was satisfactory.
