Observation: 1. Large amount of ice build-up observed in walk-in freezer on fan unit and on ceiling. Ice build-up observed on boxes of food product.
2. Electric fly trap observed stored on the top shelf above clean dishes in the dish area.
3. Handle of scoop observed touching coleslaw in top cooler well in kitchen area.
Corrective Action(s): 1. Have walk-in freezer serviced to prevent ice build-up in unit and protect food from cross contamination.
2. Move electric fly trap to a location that is positioned away from food and food contact surfaces to prevent physical contamination.
3. Ensure all in-use utensils are stored in inserts so the handles do not contact food to prevent cross contamination.

Observation: 1. Standing water observed under shelving holding boxes of pop syrup (bibs).
2. Gap observed beneath the car hop door.
3. Holes observed in the walls behind equipment in the kitchen and in the dish washing area.
Corrective Action(s): 1. Remove the standing water in the back storage pop area to prevent the attraction of pests.
2. Seal the gap under the car hop door, and seal the holes in the walls to prevent the entry of pests

Observation: Thick build-up of long standing debris observed under the sink in dish area, and on the floor under the mats in walk-in freezer.
Corrective Action(s): Clean area to remove long standing debris and clean regularly to prevent the attraction of pests and prevent cross contamination.
