Observation: Some mouse droppings noted:
1. behind toaster oven in front
2. Under muffin rack in back kitchen - mouse trap appears sprung
3. Under work station

Corrective Action(s): Ensure all mouse droppings are cleaned up and areas sanitized. Monitor these areas for further pest activity and point these areas out to pest control technicians upon next visit.
March 2021 inspection indicated minimal activity, send Apr 2021 PC report to FHA for review. Follow all PC recommendations.

Observation: 1. Raw wooden shelf used to hold work-site telephone
2. Raw wooden frame for protective barrier at POS
Corrective Action(s): Ensure all surfaces in premises are smooth, durable, easy to clean and non-absorbent to allow for proper cleaning and disinfecting. Raw wood does not meet this standard. Raw wood can be sealed by stain or paint or completely replaced with material that meets the standards.

Observation: Missing soap dispensers:
1. No soap dispenser mounted on wall by handsink near POS
2. Soap dispenser appears broken by sandwich prep station
Soap pump that would usually be in dispenser is available on its side.
Corrective Action(s): Ensure proper soap dispensers are available to allow for proper handwashing. Repair, replace, install dispensers at all handsinks that do not have a functioning dispenser.

Pest control report reviewed for Mach 2021 - minimal pest activity noted

General sanitation satisfactory

Covid Precautions observed:
1. Staff and customers mask use
2. Physical barrier at POS
3. Take-out only - no dine in - chairs and tables removed
4. Directional arrows - flow
5. Closed Men's and Women's public washrooms - family public washroom available but locked
6. Peroxide Multi Surface Disinfectant and Cleaner product used on non-food contact surfaces
7. 1 person on break at a time

Recommendations:
1. Close all public washrooms - direct public to the mall
2. While dine in is closed to public change line up on left side as you enter to keep those in line away from those leaving 3. Add occupancy limits to rooms -break room, office, walk in cooler/freezer
4. Remove extra chairs (4) from break room down to 1 - allows more space to move and eliminates seating for additional people
5. Add covid signage to through out facility for public and staff areas
6. Seal wooden frame at barrier near POS as noted above
