Observation (CORRECTED DURING INSPECTION): Back kitchen - A pail of raw bean sprouts stored directly on the floor. The pail relocated at this time.
Front kitchen - A box of green beans stored directly on the floor. The box relocated at this time.

Observation: Observed dead cockroaches on glue traps located in 2nd floor storage area. Routine pest control service in place.

Observation: Throughout the facility - Observed flattened cardboard boxes.
2nd floor food storage area - Sanitation improvement required (dried food debris and dirt accumulation). Near wall to floor junctions, and especially in between chest freezers.
Corrective Action(s): Discard all the flattened cardboard boxes. Clean and sanitize areas described in the second floor. Correct this by: immediately.

Routine inspection conducted:
Hand washing stations are fully equipped with hot and cold running water, liquid soap and paper towels.
General sanitation at the front serve area and the kitchen is adequate. Sanitation improvement required in the 2nd floor storage area (refer to the violation section for more details).
Low temperature dishwasher's sanitizing cycle measured at 100 ppm chlorine at the plate level.
Hot holding (rice cookers, hot water baths, etc.) measured at 60C and higher.
All coolers measured at 4C or less.
All freezers measured at -18C or less.
Foods are adequately covered and protected (refer to the violation section for an exception)
