Corrective Action(s): Remove all cups with no handles from storage bins. Ensure a scoop with handle is used and stored outside the bin in a sanitary manner to prevent cross contamination of food. Date to be completed: immediately.

Observation: 1. Cardboard used to line shelving under grill and shelving in dry storage area.
2. Clear tape used to repair tiles on wall next to sand witch cooler in kitchen area.
Corrective Action(s): 1. Cardboard is not to be used for lining shelving for it is not durable, nontoxic, and easy to clean. Remove all card board from shelving to maintain the premises in sanitary condition and prevent the attraction of pests.
2. Remove all tape from tiles for it is not durable, nontoxic, and easy to clean. If tiles are damaged repair/replace tiles so they are durable and maintained in good repair. Date to be completed by: immediately.

Observation: Wooden shelving observed covering mop sink in kitchen area.
Corrective Action(s): As approved plans remove wooden shelving from covering the mop sink. Mop sink is to be available for removing waste water from cleaning. Wood shelving collects moisture and is not easy to clean. Date due by: immediately.
