The manager on site indicated that this is due to a leak in the men's washroom on the other side of the wall. Corrective Action(s): Ensure that the wall is fixed with a smooth, easy to clean material within one month. Violation Score: 5

Comments

All coolers were < 4 C. All freezers were < -18 C. No evidence of pests, pest control comes on site monthly. Quats sanitizer was at 200 ppm. Hand sinks supplied with hot and cold water, soap and paper towels. General sanitation was satisfactory.
