HAZARD RATING FOR YOUR FACILITY: Low (≤15) Total Score = 3
Critical Hazards: There are no critical hazards.
Non-Critical Hazards: Total Number: 1
101 - Plans/construction/alterations not in accordance with the Regulation [s. 3; s. 4]
Observation: 1) There are some loose/cracked/missing floor tiles and damaged baseboard under the back handsink that's attached to the 2-compartment sink. As per operator, this is due to water damage from the staff washroom flooding. Operator has already contacted mall management to have the issue addressed.

2) There is no garbage can in the back. Operator has moved it to the front to prevent staff from throwing food debris in it since it attracts flies.
Corrective Action(s): 1) Replace/repair the floor tiles and baseboard.

2) Provide a garbage bin in the back. You can purchase ones that have a lid that's operated using a foot pedal.
Violation Score: 3
