Observation: Permit to operate was available however it lacked the decal to validate it for this fiscal year (April 1, 2016 to March 31, 2017) and it was not posted in a conspicuous location. The decal on the permit to operate expired in 2013.
Corrective Action(s): Place the current decal on the permit to operate and post the permit to operate in a conspicuous location; Correct within 1 week.

Observation: The trolley containing dry flour bags requires cleaning to remove the buildup.

Observation: The pizza cooler lacked a thermometer inside it.

Comments

Liquid soap, hot and cold running water, and paper towels were available at the handsinks in the kitchen and washroom. *Please add another garbage bin near the handsink in the kitchen or move the existing garbage bin in the kitchen closer to it.
The operator was washing their hands before preparing pizza.
Pizza cooler was at 0.7 degrees C (4 degrees C or less).
Walk-in-cooler was at 3.5 degrees C (4 degrees C or less).
Chest freezer and upright freezer were -18 degrees C or less.
100 ppm chorine sanitizer was available inside the bleach bottle. The person in charge mentioned that they use paper towels to wipe the sanitizer on surfaces.
Drain plugs were available for the 3-compartment sink. Reminder: Ensure food containers, pizza trays, and knives are washed, rinsed, sanitized (with 100 ppm chlorine sanitizer or 1/2 teaspoon bleach per L of water) with a 2 minute contact time, and then air dried.

NOTE: The food safety plan and sanitation plan was not available on-site. Ensure these plans are available on-site by Monday (December 12, 2016). Provide an update to the area E.H.O. regarding the status of these plans early next week.
