Observation: Noticed evidence of mice activity underneath the shelves, behind the upright cooler and in dry storage areas. Opeator is aware of the issue and has started monitoring and control program.
Corrective Action(s): 1. Ensure to clean and disinfect (1000 ppm bleach) all affected areas to remove all mice droppings. (By May 29, 2022).
2. Ensure to clean and sanitize all big pots, utensils and equipment
3. Hire a licensed pest control company as needed for pest control efforts. (

Observation: Noticed several holes in the ceiling resulting from running electrical wires and plumbing lines through the ceiling.
Pizza boxes stored on the floor leaving small gap agains teh wall which may result in mice harborage (hiding areas).
Corrective Action(s): 1. Seal or patch all holes in the walls and ceiling (By May 30, 2022).
2. Remove all cardboard or loose/ripped vinyl coverings from the wooden storage shelves.
3. Ensure that box used to store pizza pans are kept covered overnight to preven access by rodents.
4. Replace the exisiting screen door at the back entrance with a properly fitted screen door.
4. Improve general sanitaiton to ensure no food or water is available.
