Observation: Mice droppings noticed throughout the premises including underneath the shelves in dry storage area, around dough mixer and underneath pizza sheeter.
Corrective Action(s): Remove all mice droppings immediatley. Thoroughly clean and sanitize all areas of the kitchen and dry storage area including floors and equipment. Dismatle all equipment for through cleaning. Sanitize all equipmnent, sheving and surfaces with 200 ppm chlorine bleach solution. (Immediatley)

Observation:
Corrective Action(s): Following actions must be taken immediately.

1.Remove the storage pallets with shelves at least 12 inches off the floor.
2. Seal and patch the gap in dry wall ( plumbing conduit) to prevent harborage of mice.
3. Move all equipment including bar coolers, dough mixers, upright freezer, prep cooler and other items to check for droppings.
4. Seal and patch any holes or gaps in the walls that is at least the size of a quarter.
5. The floors must be thoroughly cleaned and santiized at the end of every day to elliminate food for rodents.
6. Install screen door at the back entrance ( By June 20, 2018.
7. Replace or wash the blanket cover used for proofing pizzas.

Comments

Correction Order issued.
The operator agreed to close the restaurant voluntarily untill the cleaning tasks are completed.
A detailed pest control inspection report must be submitted to our office by June 21, 2018.
ENSURE ALL ITEMS AND EQUIPMENT ARE STORED AT LEAST 6 INCHES OFF THE FLOOR TO ALLOW
FOR EASY MONITORING AND CLEANING.

A detailed revised sanitation plan must be submitted by June 21, 2018 outlining:
1. Daily cleaning and sanitizing routine for floors, equipment, shelving and other surfaces
2 List of cleaning agents and sanitizers to be used for clean up

Failure to address the above issues will result in a CLOSURE ORDER and Further enforcement action.
