Observation: Shelving above dishwasher and vegetable wash sink is made of raw wood and plywood surfacing. Corners and surfaces have been repaired by tape over time. Tape and laminate are chipping and peeling in strips or pieces.
Corrective Action(s): Repair or resurface with cleanable materials.
Do not use materials that can chip or peel off into food.

No signs of pests at the time of inspection - some traps are kicked out of place or placed flipped the opposite way.
