Corrective Action(s): Ensure that cold potentially hazardous foods are stored/displayed at 4C or below. Operator placed inserts in cooler or on ice to assist with maintaining temperature during orders.

Operator found liquid hand soap during inspection. Ensure an adequate supply of liquid soap available for replacing/refilling.

Operator relocated onions during inspection.
