Was advised by staff, sink is not used. Corrective Action(s): - Must remind all staff sink is to be used to wash hands before handling clean dishes and food. Sink in front is not easily accessible for the staff working in the back. - Ensure all sink are well stocked with liquid soap, and paper towel Violation Score: 5

Store all personal items in designated locations.
