Observation: Paper towel dispenser for handsink not functional, paper towels were found placed on dish rack on top of dishwasher along with the liquid hand soap.
Corrective Action(s): Both liquid hand soap AND paper towels must be directly at the handwashing stations. They should not be placed on a dish rack, as this can cause staff to contaminate surfaces/utensils/dishes when grabbing the paper towels and soap.

Re-install mount for dishwasher or install some form of paper towel mount (a spool on the wall is acceptable) such that the the paper towels can be dispensed in individual sheets and staff don't have to grab the entire roll. Also, the liquid soap must be placed at the hand sink so staff are not washing dirty hands and reaching over into another area with the dirty water dripping onto surfaces/equipment.

This was previously noted on an inspection report. As this is your second warning now, a Correction Order has been issued. You have been given 1 month to get this corrected. If upon re-inspection these items are not in place, you will be fined/ticketed for violation of a health act order and not having an adequate handwash station.
