Observation (CORRECTED DURING INSPECTION): 1. Raw meat product in shallow pans was stored above ready-to-eat food product in the walk in cooler
2. Raw seafood product in a food grade container was stored above ready-to-eat vegetable product in the back single door upright cooler
Corrective Action(s): 1. Walk-in cooler was reorganized such that all raw product was on one side or stocked on the bottom shelves
2. Single door upright cooler was reorganized such that the raw seafood was stored on the bottom shelf

Observation: 1. Observed 10+ mice droppings on the second floor storage area, between freezer units and underneath shelving
2. Observed cockroaches on sticky traps on the second floor storage area underneath dry storage
3. Observed 10+ mice droppings near back door entrance where rice was stored. (CORRECTED).
Corrective Action(s): 1. Ensure that the above mentioned areas on the second floor storage area are adequately cleaned and sanitized to mitigate the presence of pests. (corrective date: Immediately)
2. Continue with licensed pest control service interventions
3. Storage area near the back door entrance was cleaned and sanitized during the inspection (CORRECTED)
4. Rice near the back entrance was VOLUNTARILY discarded by the operator
5. Increase frequency of cleaning and sanitizing of the second floor. Update sanitation plan to reflect increased cleaning frequency.
