Observation (CORRECTED DURING INSPECTION): - Food debris accumulation and mold-like substance observed on exterior of smoothie machine dispenser spout.
Corrective Action(s): - Clean and sanitize smoothie machine dispenser spout. [Correction Date: Immediately]

Note: Smoothie machine not in use at time of inspection. Employee advised that a leak was discovered yesterday and machine is scheduled to be serviced today.

Observation: - Rodent droppings observed in upstairs storage area.
Corrective Action(s): - Remove droppings and clean upstairs storage area.
- Ensure that spilled food debris and old packaging materials are removed frequently to prevent potential pest attraction and access to nesting materials.
- Ensure that all areas of the premises are accessible for pest control service management. Follow recommendations provided by your pest control service technician.

Observation: - The following areas require thorough cleaning:
- Rodent droppings along wall edges in upstairs storage room.
- Miscellaneous items and debris accumulation under and between storage shelves in upstairs storage room.
- Grease accumulation under cooking equipment along deep fryers line.
- Garbage and food debris accumulation under and behind equipment in beverage preparation area.
- Grease accumulation between equipment in beverage preparation area.
- All other surfaces and hard to reach areas.
Corrective Action(s): - Thoroughly clean above noted areas.
- Review easily/ commonly missed hard to reach areas with maintenance employees so that they are also part of the regular cleaning schedule.

Comments

- Handwashing stations are accessible and provided with hot and cold water, liquid soap and papertowels to facilitate handwashing practices.
- Proper hand hygiene and handwashing practices observed at time of inspection.
- "Blue glove" use and procedures for handling raw hamburger patties reviewed with assistant manager. No concerns noted, food handlers appear to be following procedures.
- Food handlers wear hairnets and uniforms. Uniforms appear clean.
- Coolers and cold holding equipment are operating at acceptable temperatures: ≤4C
- Freezers: -12C to -19C (Food is maintained in a frozen state).
- Hot holding equipment are maintaining food at acceptable temperatures: >65C
- Food storage practices appear satisfactory. Storage is organized and food is protected from contamination.
- Employee demonstrated that Daily Food Safety Checklist completed earlier today.
- Interior of ice machine and ice bins appear maintained in a sanitary condition. Ice scoops and bulk ice bucket are stored in a sanitary manner.
- Wiping cloths stored in 100 ppm chlorine bleach sanitizing solution.
- 200 ppm QUATS sanitizer available from dispenser for manual warewashing and sanitizing purposes.
- 200 ppm QUATS sanitizer available in spray bottles at food preparation areas.
- Low Temperature Chemical Sanitizing Dishwasher: 100 ppm chlorine residual.
- Chlorine and QUATS test strips available to verify sanitizer concentrations.
- General sanitation (unless otherwise mentioned) appears satisfactory.
