Observation (CORRECTED DURING INSPECTION): Found large insert of rice cooling behind cans of pineapple and pop drinks.
Corrective Action(s): Must cool foods properly in shallow pans, preferably on ice.

Observation (CORRECTED DURING INSPECTION): Found potatoes in cooler to be 10 C, Chicken to 7 C.
Corrective Action(s): Must keep potentially hazardous foods below 4 C at all times. Discard items. Keep small amounts on ice in top portion until cooler repaired.

Observation (CORRECTED DURING INSPECTION): Found pork dish in the front hot holding unit to be 48 C after 1 hour.
Corrective Action(s): Must keep potentially hazardous foods above 60 C at all times. Reheat to 74 C and keep above 60 C.

Observation (CORRECTED DURING INSPECTION): 1. Found multiple inserts put away with water pooling in the bottom, or stacked upside down with water trapped inside.
2. Found wiping rag in back prep area to have 50 ppm chlorine.
Corrective Action(s): 1. Remind all staff of proper manual dishwashing: i.e. dry inserts completely upside down before stacking.
2. Ensuring wiping rag is kept in 200 ppm chlorine.

Observation: 1. Found boxes of produce on the floor at time of arrival.
2. Found a bag of onions on the floor in behind crates, and under floor broom.
3.Found food containers inside facedown on the floor
4. Found multiple dented cans (i.e. tomato sauce, pineapples, coconut milk) being stored for use if not already being used.
Corrective Action(s): 1. Box was lifted off of the floor
2. Discard onions as unsure of what contaminates have leached into the onions
3. Must keep all food and food contact surfaces 6" off the floor at all times.
4. Discard all product that is in dented cans. Do not purchase or accept deliveries of such product.

Observation: Heavy build up of grease and food debris throughout premises, including but not limited to under hard to reach areas, on wire shelves, walls around food prep, cooking and dishwashing stations, ventilation hood, handles and food contact surfaces.
Corrective Action(s): Extensive cleaning is required through out facility including but not limited to the above noted locations.

Observation: Found corrugated plastic lining shelves, repeat observation.
Corrective Action(s): - Do not use corrugated plastic to line shelves, it is not easily cleanable and provides a home for pest.
- Remove all pieces of plastic from premises and increase cleaning on wire shelves instead of cleaning them up.

Observation: Prep cooler was 8 C, internal food temperatures were 7-8 C
Corrective Action(s): Have unit repaired/replaced so it can maintain 4 C at all times including busy rushes.

- Walk in cooler was 2 C
- Glass door cooler was 3 C
- Freezer was - 10 C
- Hot holding was 60 C or hotter with the exception of the above noted area
- No signs of pest activity noted during inspection

NOTE: if unable to operate in a sanitary manner, may need to consider reducing menu items.
