Observation: Cockroach bodies on sticky traps under sink in staff washroom.
Cockroach, dead, behind toilet and behind 1 upright freezer.

Owner mentioned there was some live sightings in November 2023 and claims these are old.
Owner purchased 3.78L bottle of Permethrin 0.35% and applied in December 2023 to the washroom, behind the freezer and underneath the dishwashing areas when the restaurant was closed.

Corrective Action(s): 1. Sweep, remove, clean and sanitize all areas identified: washroom, under the freezer.
2. Stop the application of pesticides in the food preparation area. Sticky traps that are specific to this pest can be placed in problematic areas to monitor.
3. Call a pest control contractor to inspect, treat and monitor; follow up with your pest control contractor.

o Operators had clean clothing for food preparation
