Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care and Assisted Living Act (CCALA), the Child Care Licensing Regulations (CCLR) and the relevant Director of Licensing Standards of Practice (DoLSoP). All care categories were inspected for compliance.

Observed Violations
LICENSING: 10010 - CCLR s.10(1) - Applicants for licences and licensees must notify a medical health officer immediately of any change in the information provided under section 9.
Observation: Observed School-age Program ( 8 children ) participating in activities in the kitchen which is licensed/ designated for the Multi-age program. 5 Multi-age children played in a room separated by a baby gate entering the kitchen once the school-age program was done. Observed a playpen in the school-age room which is used by a child attending the Multi-age program while School-age children are in attendance. Provided copies of stamped Floor Plans along with this inspection report The School-age program includes a separate kitchenette and washroom in order to support that programs are to operate separately. This was a requirement in order to amend the licence and add the additional care program. The entrance area is the only common area where all children may enter and then proceed into their separate programs.
Corrective Action(s): Please forward your plan of how you will ensure programs do not operate concurrently

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11090 - CCLR s.15(1)(b) - A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.
Observation: In review of the fire plan it is noted that the garage door is marked as an emergency exit, many stored items by the entrance door hamper the exit.
Corrective Action(s): please ensure exits remain free of items for safe evacuation.

STAFFING: 12050 - CCLR s.19(1)(b) - A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation: 3 staff files reviewed contain a check mark under the word references however references were not located or listed on the file
Corrective Action(s): please ensure references are obtained and checked for each staff working at the facility.

RECORDS AND REPORTING: 19030 - CCLR s.55(2)(a) - A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.
Observation: Incident documented in the internal log book , family sought medical care for their child. The incident was not reported to Licensing. LO spent time reviewing reporting requirements with staff.
Corrective Action(s): Please submit a Reportable Incident Form to Licensing and provide your plan to ensure staff are aware of reporting requirements.

RECORDS AND REPORTING: 19360 - CCLR s.60(5) - A licensee must keep a record referred to in section 57 [records for each child] for at least 2 years from the date the child who is the subject of the record is discharged from the community care facility.
Observation: In review of children's files it is noted that site specific locations are not listed some children attend more than one location. One file was not available for review as requested as the child now attends another location.
Corrective Action(s): please ensure children's records are maintained and avaible for review 2 years from date of withdrawal.It is required that Children's records are kept current reflecting the facility they attend.

Comments

At the inspection dated July 20, 2021 there were 13 children and 2 staff present. The facility manager was contacted and arrived on site during the inspection. The manager no longer works scheduled shifts at the facility but rather floats in-between several centers providing assistance to staff. Staff informed Licensing that a new manager is being put forward as the School-age Program officially opened it's doors on July 1, 2021. Staffing changes recently occurred at another location , Licensee Contact to submit Licence to Amend Form for Change of Manager at 2 locations.
Please note that past discussions of a required Food Service Permit would now apply to this location as the issued capacity exceeds 8 children. It is noted that cooked foods are provided to children such as hot dogs. Please advise how you wish to proceed as a referral to an Environmental Health Officer ( EHO ) will be required.
At this inspection the above noted contraventions were identified. As part of this Routine Inspection a Facility Risk Assessment Tool is completed and a copy is provided.
Inspection report completed off site therefore no signature obtained.
