Observation (CORRECTED DURING INSPECTION): 1. Reach in glass cooler ambient temperature measured at 8 degrees C. Internal temperature of potentially hazardous foods (cooked meats, liquid egg, noodles, and gravy) measured 6 to 7 degrees C. Items were transferred to cooler at 4 degrees C or less at time of inspection.
2. Cooked noodles with packaging that states "keep frozen/refrigerated" were found stored at ambient temperature in hallway. Internal temperature measured 20 degrees C. Discarded.
Corrective Action(s): Please ensure cooler is adjusted or repaired to maintain 4 degrees C or less in order to limit the growth of potential pathogens and/or toxins. Do not store potentially hazardous foods in this cooler until such time. Please ensure potentially hazardous foods are thawed properly (e.g. cold running water, cooler at 4 degrees C or less).

Observation (CORRECTED DURING INSPECTION): Kitchen hand washing station was filled with pans and utensils at time of inspection. Items were removed by staff. Ensure that no items are stored in the desingnated hand washing station as it must be available at all times to facilitate proper hand washing.

Observation: 1. Newspapers were observed covering mushrooms in walk in cooler.
2. Oil soaked cardboard observed on lower shelves in cooking area.
Corrective Action(s): Newspapers removed at time of inspection. Ensure that only food grade materials are used to cover foods to protect from potential contamination (e.g. ink). Please remove all cardboard from the premises. Ensure that all surfaces are smooth, impervious to moisture, and easy to clean.

Observation: Thermometer located in glass cooler reads "100 C."
Corrective Action(s): Please provide a working thermometer accurate to 1 degree C for this cooler in order to check temperatures. Date to be corrected by: December 11, 2017.

Routine inspection conducted:

- Walk in cooler at 3 degrees C. Food is covered and well organized.
- Prep cooler at 2 to 3 degrees C (above) and 2 degrees C (below)
- Small reach in freezer at -23 degrees C
- Walk in freezer at -15 degrees C
- Hot holding internal temperatures (steam table inserts, rice) at 60 degrees C or more
- Hand washing stations (front, kitchen) supplied with hot and cold running water, liquid hand soap, and single use paper towels.
- Wiping cloths stored in bleach water sanitizer at 200 ppm chlorine. Three compartment sink set up for manual warewashing -- 200 ppm chlorine available in third compartment.
- Ice machine maintained in sanitary condition. Scoop stored safely.
- No evidence of pest activity at time of inspection. Please forward most recent pest control report to inspector.
- FOODSAFE requirements met at time of inspection.
- Permit posted

Notes:
1. Temperature records were dated to December 11th. Please ensure that temperatures are recorded daily for the correct date. Infrared thermometer is available for temperature checks.
2. Glass cooler is not listed as part of temperature records--discussed with staff. Column to be added to record glass cooler temperatures.
3. Please ensure storage spaces are organized and any items not required for business purposes are removed from the premises. Space is limited.
