Comments

The following corrective actions have been taken: - Mouse droppings, dead mouse, gnawed materials, and loose package of bait have been cleaned up/removed. - Pest control technician attended premises to conduct additional service which included an access point investigation. Copy of report was provided to EHO. Next service is 17-Nov-2022. - Identified holes/entrance points addressed - New door sweep installed - Coving at the back replaced with tile. Coving in washroom repaired. - Back hallway has been organized and items are stored off of the floor. - Cleaning of shelf under hot holding unit has been completed. The following issues remain pending and require correction: (Refer to routine inspection report 31-Oct-2022 for violation observations and corrective actions) - Code 304 - Provide copy of Pest Control Contract to EHO - Code 309 - Bait boxes with rodenticide remain present in washrooms and back hallway. Remove from premises; replace with traps. To be completed by: today - Code 311 - Flooring under rationale ovens requires repair/replacement.
