2. Large hole in wall in back left corner of back room behind chest freezer. Hole used to access water valve in wall.
3. Missing covers over light fixtures.
Corrective Action(s): 1.Ensure sanitizing step is used in 3 comp sink once cycle is completed

2. Ensure large hole in wall is covered to prevent possible pest entry into premises. Cover can be removable if access to valve is required.
3. All lights must have covers or protective sleeves installed to protect food. Install protective covers.
