Observation:
All bait boxes that were identified in the last inspection that were required to be removed are still in place. Significant number of old and recent mouse droppings found throughout kitchen and storage area. Operator has added glue boards since inspection.
Corrective Action(s):
Remove and discard all bait boxes immediately. Bait must NOT be used anywhere inside of restaurant. Deep clean entire restaurant area. Only mechanical traps can be used in restaurant (for example glue boards, snap traps and live traps). Plug all holes where mice could go through walls or enter/move through equipment. Contact pest control company if problem with mice continues.

Observation:
Build-up of food debris, grease, mouse nesting materials and mouse droppings observed throughout kitchen area and storage room.
Corrective Action(s):
Deep clean kitchen area, all food storage areas and behind/under all equipment removing everything written above. Cleaning must be done as soon as possible, operator requested two weeks for completion of cleaning.
