Observation: 1) Back hand sink paper towel dispenser doesn't work, no paper towels out to use
2) Front hand sink blocked by lid in the sink
3) Front hand sink has no paper towels in the dispenser, only paper towels by cash register. Staff cannot open the paper towel dispenser due to a lost key
Corrective Action(s): Ensure both hand sinks have no items in them and both have liquid soap and paper towels to allow for proper hand washing by Monday March 18/24

Observation: Cockroaches found at the back of the restaurant among the clean linen bags and boxes of take out containers.
Corrective Action(s): 1) Have pest control company come in and spray for cockroaches. Email EHO when company can come in for first treatment
2) Clean and sanitize all areas paying particular attention to the food contact areas
3) Declutter to allow for ease of cleaning.
4) Place all food and take out containers in pest proof containers to ensure they don't become contaminated by Monday March 18/24

Observation: Floor tiles by stand up cooler have become progressively cracked and are now lifting and coming apart.
