Observation (CORRECTED DURING INSPECTION): Sushi rice is stored for up to 2 days at room temp. pH is bettween 4.5 and 5.
pH is not low enough ton ensure shelf stability, and furthermore pH can change over time.
Submit a food safety plan for sushi rice which details pH, and holding times for sushi rice, also cooling times, and how cooling and holding times are tracked. Food safety plan for this item is required within one week.

Observation: 1) Holes noted in ceiling and floor of ice machine room. Use rodent proof materials to fill/cover holes. Access panels/covers must be secured.
2) Damaged/moldy dry wall around mop sinkcould provide harbourage for rodents. Repair.
3) Pooled water noted under dishwasher & floor near mop sinks/ice machines. Repair any leaks and take care to keep water from pooling.
