-Follow-up inspection to Sept 29, 2016.
-Facility has retained Atlas Professional Pest Control services. 9 Tin cats and 37 snap traps have been set up throughout the facility. Facility will be monitored on a weekly basis until pest infestation is under control. After the pest issue is under control, pest control operator will evaluate inspection frequency. Ensure facility implements recommendations of the Professional Pest Control operator to prevent reinfestation of the kitchen.
-Facility has completed cleaning and sanitizing of affected equipment and utensils.
-All equipment and utensils have been rewashed and sanitized in the chemical dishwasher.
-Conditions of the Public Health Act Closure Order have been satisfied. Closure Order has been rescinded, facility may reopen for service.
