Observation: live cockroaches found behind the dishwasher, and around the hot water connection to the dishwashing sink. These areas continue to be a problem.

Corrective Action(s): Discussed with operator that diligent trapping, spraying and cleaning is required around this area.
Operator will be fumigating the whole facility Monday with a new pest control company.

Further cleaning of the facility conducted. Bar area, canopy top, upright cooler fan, and behind the basement kitchen counters.
Continue to monitor and clean the dishwashing area and furnace area (basement) as these are high activity areas. Facility to be fumigated Monday, March 08.
Pest activity noted to be significantly decreased. correction order rescinded.
At the end of the night daily, operator will be bagging/sealing all dirty laundry, removing all garbage, putting away all food items, and conducting a thorough clean.
Continue to use sticky traps to monitor for cockroach activity and identify high risk areas.

Operator must provide inspector with follow up report from pest control professional after fumigating. All recommendations must be followed.
Pest control should be biweekly servicing.
