Observation (CORRECTED DURING INSPECTION): Ice machine is not functioning properly AND the internal/external components dirty. The ice tray inside is allowing the growth of biofilm which is present on and around the ice-making trays. The external parts of the unit, including the door is dirty and breaking.
Corrective Action(s): Do not use this machine for any ice purposes. Obtain ice from third party until:
- unit is repaired including door mechanism.
- unit is cleaned and sanitized inside and out.
UNIT WAS TURNED OFF.

Observation: prep cooler at 9 C. Indicating thermometer not accurate as was measuring 0 C.
Corrective Action(s): Immediately move all potentially hazardous foods from this unit.
Replace thermometer with one that is accurate and that functions.
Make necessary adjustments to unit and/or call service technician to repair. Make sure unit always maintains temperature of 4 C or less at all times.

Observation: Parts of the kitchen observed with excessive build-up of food debris. These include:
- area below grill and deep friers - mats covering floor underneath equipment impeding proper clean up.
- Shelves immediately around the cook line - again, mats impeding proper cleaning.
- Display cooler tracking and interior cavity - Track is breaking apart and not cleaned well enough.
Corrective Action(s): Clean up identified areas. Check at follow up inspection date noted on report.

Observation: - Rusted shelves in walk-in cooler observed.
- Tracking on display cooler broken, inhibiting both cleaning and proper closing.
- Magnetic door seal on prep cooler falling apart and is both difficult to clean and keep door properly sealed.
- Floor is in poor condition. Paint is peeling and floor is pocked.
Corrective Action(s): Consider plan to rehabilitate this premises and fix deteriorated components of facility as it is affecting the ability to keep facility clean and safe for public.
