Observation (CORRECTED DURING INSPECTION): In-use wiping cloths on the cook line and in the dining area were not stored in the respective buckets of bleach sanitizer in between uses.
Corrective Action(s): In-use wiping cloths must be stored in a bucket of bleach sanitizer in between uses to prevent bacteria from growing on them. Instructed staff to correct violation at time of inspection.

Observation: 1. Mildew found on large wooden rice paddle.
2. Small chest freezer in the back storage area is filthy.
Correction: 1. Large wooden rice paddle used to mix sushi rice was found to have mildew on it. Also, the surface of the paddle, especially around the handle area is no longer smooth. The wooden paddle appeared as though it can no longer be effectively cleaned and sanitized, and shall be removed from use immediately. The rice paddle shall be replaced with a new one that is smooth, easy to clean and does not absorb moisture. Wooden rice paddle was discarded at time of inspection.

2. The interior of the small chest freezer shall be thoroughly cleaned to prevent contamination of food.

Follow up inspection.
Large wooden rice paddle noted in previous inspection report has now been replaced with a new plastic rice paddle.
Small chest freezer has been cleaned.
All work areas are equipped with a bucket of bleach sanitizer (i.e. sushi bar, 2 along cook line, and one in the dining/servery) and all had chlorine concentrations of 200ppm.
Cooked tapioca was covered.
Mouse droppings noted in previous inspection report have been cleaned up.
