Corrective Action(s): Do not use mop sink hose to fill pots for food, use clean side of 3 compartment sink, may need to install prep sink to accommodate safe food handling.

- food was stored 6" off the floor
- food was not being thawed at room temperature
- hand sink had liquid soap, paper towel hot and cold water
- newspaper was not found in kitchen area
NOTE: meat slicer still had meat pieces up in the top well, must clean entire unit every 4 hours or after each use. Corrected during inspection.
NOTE: all equipment must be washed and sanitized every 4 hours purchase more utensils in order to have 1 set in use while the other set is being washed.
