Corrective Action(s): Directed operator to refresh the bleach sanitizer so there is 100ppm chlorine available.

Observation: 1) A grocery bag was used to store frozen wontons directly. 2) Inquiring operator about frozen tofu and a piece fell to the ground. Operator picked it back up and placed it back to the container full of tofu.
Corrective Action(s): 1) Do not use grocery bags to store food directly. Use food grade bags only. 2) Frozen tofu discarded. Do not use food if it has fallen to the floor as the floor is dirty.

Observation: Mice droppings and silverfish on glueboard were noted behind chest freezers in the dining area.
Corrective Action(s): Please remove droppings and silverfish and properly clean this area.

Observation: 1) Ceiling by cooking stove is observed to be peeling. Operator informed that there was water damage from air conditioner next door. 2) New chest freezer added in the dining area.
Corrective Action(s): 1) Repair the ceiling so the surface is smooth and can be easily cleaned. 2) No concerns this time but do not make structural changes in the future without Health Approval.
