Observation: Multiple food contact surfaces not maintained in sanitary condition. These include but are not limited to:
- Plastic food bins, work surfaces, proofer, Cooking sheets and trays etc.
Corrective Action(s): clean, sanitize and properly store all tools, equipment and utensils in premises.

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation: Proper dishwashing procedures are not beig followed. Proper 4 step process is lacking. Use of sinks in the front for dishwashing is not acceptable.
Corrective Action(s): Ensure proper wash rinse sanitize (using proper sanitizing agent properly) and air dry is completed in a manner that is acceptable. Dishwash area lacks proper organization to complete this sanitary function.

Observation: Cleaning cloths for surface and spot cleaning and sanitizing observed dirty without any evident system to ensure they are sanitized before and after use.
Corrective Action(s): Implement a "red" bucket system to keep and use for cleaning cloths to use for surface sanitizing.

304 - Premises not free of pests [s. 26(a)]
Observation: Observed rodent droppings, both mice and rats in premises. Observed rat traps in a pile under shelves. Observed drain flies in several areas of premises. Observed wood mites on counter immediately next to double sink in front of premises.
Corrective Action(s): Eliminate all pests infesting premises. Enter into contract with a Pest Control technician to set up pest abatement accordingly with above noted pests. Eliminate any entry points. Immediate action required.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Many areas of premises suffering significant accumulation of food and related debris and there are areas of the premises that lack any consistent cleaning schedule as required. This include but again are not limited to:
- Floor in back prep area;
- shelves in dry food storage, walk-in cooler and freezer;
- back door area;
- office - office is leaking into food prep area of store;
- below dish sinks; the sinks themselves;
- mop sink;
- behind oven;
- Floor of walk-in cooler/freezer;
- doors and door handles;

Corrective Action(s): Thoroughly clean above noted areas;

Observation: Area next to front double sink deteriorated. Dripping leaking accumulation of water on floor of walk-in cooler (due to condensation) observed.
Corrective Action(s): Repair above noted items. Develop plan to replace counters and/or shelves that are deteriorated and submit to this office.

309 - Chemicals, cleansers, & similar agents stored or labeled improperly [s. 27]
Observation (CORRECTED DURING INSPECTION): Quats in spray bottle measured at over 1000 ppm.
Corrective Action(s): Properly label spray bottles to indicate Quats/water ratios. Develop sanitation plan that properly identifies and incorporates sanitary processes into all aspects of cleaning. This must be in writing and submitted to this office for review.
