Observation (CORRECTED DURING INSPECTION): An internal temperature of spring rolls in the hot holding unit was measured at 50.1C. Spring rolls cooked 25 minutes ago. Operator reheated spring rolls to 74C internally at this time.

Observation (CORRECTED DURING INSPECTION): Observed a scoop for rice stored in an unsanitary condition. The scoop was washed and sanitized at this time and placed in to an empty clean container.

Observation (CORRECTED DURING INSPECTION): Observed the following:
1. (REPEATED VIOLATION) Food contact surface sanitizer was not available. 100 ppm bleach solution prepared at this time.
2. Operator demonstrated manual dishwashing of an utensil. Sanitation step was missing. Corrected at this time.

Observation (CORRECTED DURING INSPECTION): (REPEATED VIOLATION) Observed a box of green beans stored directly on the floor, and being trimmed. Box relocated at this time.

Observation: Observed the following:
- Live cockroaches caught on sticky traps (observed ~10 cockroaches per sticky trap pads) in the second floor storage area.
- Number of flies in the back kitchen.
- Number of fruit flies in the front service area.
Corrective Action(s): Dramatic reduction in pest activity is required. Work with your pest control service. Improve general sanitation. Remove any standing water. Block and seal areas where pest can enter (e.g., back door, cracks and crevices). Correct this by: 1 month.

Observation (CORRECTED DURING INSPECTION): 1. The back screen door is not effective to block pest from entering. Gaps between the door and door frame must not exist. The back door was closed at this time.
2. The way dry foods (raw rice, oil, etc.) are stored prevents a health inspector and pest control service technician to assess the pest activity.
3. General sanitation improvement required. Observed sticky residues on walls that may be attracting flies.
Corrective Action(s): Repair the screen door. Reorganize food storage area for easier accessibility. Improve general sanitation.
Correct this by: 2 weeks.

Routine inspection conducted:
Hand washing stations are fully equipped with hot and cold running water, liquid soap and paper towels.
Low temperature dishwasher's sanitizing cycle measured at 100 ppm at the plate level.
Hot holding measured at 60C or higher (see an exception in the violation section).
All coolers measured at 4C or less.
All freezers measured at -18C or less.

Further repeated violations may result in progressive enforcement actions such as issuing a violation ticket, etc.
