Observation: Automatic QUAT dispenser was broken at the time of inspection. Supply was almost empty and not dispensing adequately. Operator was creating QUAT solution manually. Repair and resupply needed.
Corrective Action(s): Ensure QUAT dispenser can adequately dispense 200 ppm to 400 ppm QUAT sanitizer solution and there is adequate supply for service. Correction required in two days. Failure to sanitize utensils and food contact surfaces may result in enforcement such as an Order or Closure.

Recommendation: Ensure to test sanitizer levels daily and record readings to ensure all equipment is in working order.
