Observation: Food contact surfaces/implements are not being properly washed and disinfected after use. Sanitizer dispenser not functioning properly.
Corrective Action(s): Clean and disinfect all food contact surfaces after use or on a timely manner (ie: ~4 hours) All items are to be stored in a sanitary manner. (ie: Shelving) Do not store items ontop of grease trap. Staff are required to be properly trained. Dispenser unplugged. Wiping cloths, used on food contact surfaces, are to be stored in a sanitizer at all times. (monitor/change routinely)

Observation: Food/debris noted along walls, behind equipment, ontop of oven, etc. Foul odor emanating from grease trap, floor drains.
Corrective Action(s): Clean and organize entire facility. Pull items away from walls. Store all items in designated areas. Remove all unused items to free up space. (ie: top of walk in cooler) Ensure grease trap is properly sealed. Flush/clean floor drains.
