Observation: 1. Bottle of wine and glass bottle of sparkling water stored in ice machine unit
2. Ice scooper berried in ice
3. Containers of food stored in walk in cooler with out covers
4. Open containers of dry breading products for meat stored on lower shelves - possible exposure to pets over night
5. Large sack of flour cut open and sitting 6 inch off the ground
Corrective Action(s): 1.Do not store beverages in ice machine, on going issue. Ice exposure to contaminated exterior of bottles and possible broken glass/plastic if bottles break - contaminating ice. Physical risks.

1.Under cooking equipment and shelves
2.On lower shelves
3.Gap under back door of unit to hallway
Corrective Action(s): Ensure all droppings are cleaned up and holes identified and sealed. Monitor for pest activity and report to pest control technician. Get pest control tech in more regularly until pest issue becomes controlled.
Install door sweep under back door of unit.

1.Cold water knob of 2 comp dishwashing sink broken off - wrench used in place of knob
2. Sliding door cooler noted 6 C
3. Shelving above dishwasher swinging door has broken off wall - support arms hanging down
Corrective Action(s): Ensure the kitchen is maintained in good condition at all times:
1. Repair/replace missing cold water knob
2. Ensure sliding door cooler is capable of maintaining a temp at or below 4 C. Monitor unit with own thermometer, if unit is unable to be maintained under 4 C then it must be serviced and then do not store PHF in this cooler until repaired.
3. Shelving must be in good repair - repair/replace shelving or completely remove shelving and seal wall holes.
